Universal VST3 plugins often fail to scan in Reaper. One workaround (not always 100% successful) is to delete the ‘MacOS’ folder inside the VST3 plugin.
When Reaper scans for new VST’s and detects a universal VST3, it’s also scanning the MAC section of the VST as well as the Windows.
As well as the workaround suggested by Jeff (remove the MAC section-not always easy) there is an automated solution from Davidson Multimedia. This comes in two versions MAC and Windows so that all bases are covered. Be sure to read the read me file fully before using the cleanup tool. (See below)

Warning!!! This set of Bat files runs recursively through the directory it is run from, and in all sub folders contained in the directory/folder. Ensure the VST3 and the Bat file are isolated in a folder were the directory does not extend beyond to other parts of the system.
Note: This issue can also apply to other DAW’s too. I had a similar issue with Reason V12 not always scanning universal VST’s.
About Universal VSTs
A universal plugin can run on either macOS or Windows.
SynthEdit now produces universal VST3 plugins. This means you can offer the same download to macOS and Windows users. It will ‘just work’ (not always! See above). SynthEdit universal VST3s support both Intel Mac computers and ‘Apple Silicon’ (ARM) based ones such as the M1, M2, and later.
As a result of being universal, VST3 plugins now take a little more disk space and contain more files than with previous versions of SynthEdit.
